Abstract Comparing between the effect of pain receptor blocking agent (Epicotil®) and Lubrication system (Curavisc®) after temporomandibular joint arthrocentesis in case of painful clicking temporomandibular joint was the aim of this study. This study was carried out on 20 patients 3 males 17 Females in the age range from 18 to 40 in group A and 18 to 66 in group B, all suffered from painful clicking temporomandibular joint. The Cases were designed based on clinical setting only as (anterior disc displacement with reduction) associated with pain in the temporomandibular joint. No radiographic interpretation has been taken. The affected joint was tender to palpation associated with clicking sound. The Twenty patients selected from 300 enrolled patients was taken from the outpatient clinic of Oral and Maxillofacial Surgery Department, Cairo University. The twenty patients were randomly divided into 2 equal groups according to flip coin method. group A: including 10 patients received Curavisc® (Lubrication System) after affected temporomandibular joint arthrocentesis. group B: including 10 patients received Epicotil® (Pain Receptor Blocking Agent) after affected temporomandibular joint arthrocentesis. Each group comprises single session of arthrocentesis by using the minimum amount of lavage solution (50 cc Lactated Ringer Solution), at the end of each session injecting any of the 2 comparing drugs intra-articularly in concentration of 20 mg to 2ml. The following measurements were taken on Verbal Numerical Rating Scale (VNRS) and in millimeters (mm) pre-arthrocentesis and post – arthrocentesis on the following time intervals 2, 4, and 12 weeks:- 1- Affected joint tenderness on VNRS. 2- Affected muscle tenderness, of the involved temporomandibular joint on VNRS. 3- MIO (Maximum Interincisal Opening) in mm. 4- Protrusion in mm. 5- Lateral Excursions from the effected joint toward right and left side in mm. 6- Absence /presence of shift / deflection. 7- Absence of painful clicking sounds. Complete aseptic technique has been used during the surgical procedure. Shepard Cannula had been introduced inside the joint during the surgical procedure. This study proved that Epicotil® drug pain receptor blocking agent was much more reliable and proved much more significant result more than Curavisc© drug (lubrication system) in the assessed measurement. Clicking shift/ deflection disappeared with both drugs. |
