Abstract A recent survey conducted by Mroz et al. (3) demonstrated the heterogeneity of treatment recommendations for recurrent disc herniation among 445 spine surgeons. Overall, there was a 22% and 69% probability of disagreement on the surgical treatment of first and second time recurrent disc herniation, respectively. Revision surgical treatment options include, revision micro- discectomy alone versus microdiscectomy with posterior fusion ± lumbar interbody fusion. Despite positive results with revision discectomy procedures, the optimal procedure for recurrent LDH remains controversial. When compared with microdiscectomy, lumbar fusion for primary LDH has not been shown to decrease reoperation rates at five or eleven years.
