Abstract

Background; Cesarean delivery is a commonly performed procedure worldwide. Despite improvements in balanced multimodal analgesia, there remain a proportion of women for whom postoperative pain relief and patient satisfaction are still inadequate, postoperative pain is one of the major concerns during and after cesarean sections. Post cesarean patients have an additional compelling reason to be provided adequate pain relief so that early ambulation helps in breastfeeding and care of the newborn. In addition, mobilization is a key factor in attenuating the risk posed by exaggerated thromboembolic phenomenon in pregnancy.
Aim and objectives; To investigate the analgesic effect of Levobupivacaine with ketamine in Subcutaneous infiltration on post-cesarean delivery pain, Subjects and methods; This study is a Prospective, Randomized, Double blinded, Placebo-controlled Clinical trial, was carried out at Ain Shams University Hospital Obstetrics and Gynecology department,0n 102 women were recruited to this study who were randomly divided into: group I (Levobupivacaine with Ketamine): it comprised 51 women who received a subcutaneous infiltration of levobupivacaine with ketamine. group II (Placebo group)which was the control group: it comprised 51 women who received a subcutaneous infiltration of a Normal Saline. during the time period from 1st of October 2022 (data collection)to the end of April 2023 (analysis of data & its results).
Result observed pain scores were significantly lower in group A(Levobupivacaine with ketamine)compared with group B(Placebo)at 4 hours (57.0 ± 5.5 versus 67.6 ± 7.0, respectively, p <.0001), 6 hours (45.7 ± 6.3 versus 59.8 ± 7.4, respectively, p <.0001)and 12 hours (34.5 ± 5.6 versus 50.6 ± 6.3, respectively, p <.0001).
Conclusion; The use of a combination of Levobupivacaine and ketamine in subcutenous infiltration significantly reduced postoperative pain and analgesic consumption in women who underwent elective cesarean section, compared with control infusions,
