Abstract

Introduction: Hypothyroidism is known to be associated with atherosclerosis and ischemic heart disease. The accelerated atherosclerosis in hypothyroid state has been traditionally ascribed to atherogenic lipid profile, diastolic hypertension and impaired endothelial function. The first structural change that can be detected in atherosclerosis is an increase in intima media thickness. Abdominal aorta intima media thickness measurement can be used to detect the development of early atherosclerosis due to the initial onset of atherosclerosis in the distal abdominal aorta.
Aim of the study: To assess Serum lipid profile and abdominal aorta intima-media thickness in patient newly diagnosed with hypothyroidism and their change after one year of treatment.
Patients and methods: 50 Egyptian patients diagnosed with hypothyroidism. The patients were divided into three groups according to their diagnosis: congenital hypothyroidism (CH), Acquired and CH stopped treatment / uncontrolled group. All patients were Subjected to: full medical history, auxological measures, blood pressure and clinical signs of hypothyroidism, non fasting lipid profile measurement and abdominal aorta intima media thickness measurement by abdominal ultrasound.
Results: High levels of cholesterol, triglycerides and low density lipoprotein (LDL) were found before treatment (259.64 ± 82.06, 193.94 ± 71.54 and 144.70 ± 48.46 mg/dl, respectively) and decreased significantly after treatment (216.66 ± 44.60, 165.70 ± 53.58 and 123.26 ± 29.79 mg, respectively). High density lipoprotein (HDL) level was low (47.92 ± 10.08 mg/dl) and increased significantly ( 57.56 ± 7.94 mg/dl) after treatment. Abdominal aorta intima media thickness decreased significantly after treatment of hypothyroidism and normalization of thyroid profile (1.47 ± 0.49 and 1.33 ± 0.41 mm respectively).
Conclusion: Upon treatment of hypothyroidism, cholesterol, triglycerides and LDL levels decreased significantly, and HDL increased. Abdominal aorta intima media thickness decreased significantly after treatment of hypothyroidism and normalization of thyroid profile. Abdominal aorta intima-media thickness measuring is an easy feasible and accepted way for early detection and diagnosis of atherosclerosis.
