Abstract

It is currently recommended that elective caesarean section should be deferred to 39 weeks. However, approximately 10%-15% of woman planed for c/s may deliver before 38 weeks, and there may be concern on waiting in the presence of specific indications or previous history.
There is conflicting advice from international organizations. RCOG recommends administration of antenatal corticosteroids to all pregnant women for whom an elective caesarean section is planned prior to 38+6 weeks of gestation. RANZOG supports this by stating, where delivery by caesarean section (without prior labor) is planned before 39 weeks gestation, consideration should be given to the administration of corticosteroids to reduce respiratory morbidity in the newborn. However, other international bodies such as the Society for Maternal Fetal Medicine (SMFM) do not advocate the routine use of steroids before early term (ET) elective CS
Also, the WAPM-World Association of Perinatal Medicine and the PMF-Perinatal Medicine Foundation, 2022 suggested that, ACS is not routinely recommended before scheduled cesarean section at term because of the current uncertainty regarding the benefit to risk ratio.
On the other hand, guidelines issued by the American Council of Obstetricians and Gynecologists, not recommend that using antenatal corticosteroids before the 39+ 0 weeks.
This was a prospective study, which was carried out on 1105 pregnant women aged 18–46 years at Ain Shams University Maternity Hospital, in Egypt. To assess the effect of routine prophylactic corticosteroids before elective caesarean section, after 37 weeks of gestational age, on neonatal respiratory morbidity.
