Abstract

The present study is a cross sectional study that included 16 aspirated hepatic cyst fluid. It aimed at eliciting the benefit of using direct microscopic examination and PCR in reaching a hydatid diagnosis for query hepatic cysts. Also, protein electrophoresis and biochemical analysis, to estimate total protein, glucose and calcium levels, were employed to distinguish hydatid from other hepatic cysts. Microscopic examination revealed 2 out of 16 samples positive for hydatid by the detection of hooks and scolices. PCR is a highly sensitive and specific method for diagnosis of infectious disease. PCR was done to all 16 aspirated hepatic cyst fluid and revealed 13 out of 16 samples (81.25%) positive for hydatid, while Protein electrophoresis revealed suggestive hydatid patterns in 14 out of 16 samples (87.5). PCR showed a statistically significant improvement in detection of hydatid disease as compared to microscopy (P value = 0.003). Also, protein electrophoresis showed a significant value as a diagnostic aid compared to using direct microscopy alone (P value <0.001). Biochemical analysis was unable to point hydatid disease. Although light microscopy is considered the most confirmatory diagnostic technique for hydatid disease, however, in this study, incorporating protein electrophoresis as a diagnostic aid and PCR has increased the percent of detectable cases significantly as the studied samples represent a sector of the Egyptian cases, establishing a comprehensive diagnostic protocol for query hepatic cysts should not be neglected.
