Abstract

Background; Preterm labour (PTL) is any delivery after 24 weeks, before complete of 37 weeks’ gestation. In preterm labour, the efficacy of tocolytic drugs has been proven to prolong pregnancy. A commonly used tocolytic drug is nifedipine which appears to be more effective than β2-adrenergic-receptor agonists and magnesium sulfate.
Aim; to compare the effectiveness of nifedipine alone, sildenafil citrate alone and combination of both in preventing threatened preterm labor.
Patients and methods; This study was conducted at El-Minia University Hospital. A total of 150 included. Patients were pregnant with gestational GA ranged between 24 and 36 weeks. They were divided into 3 groups: (group A): 50 patients received nifedipine tablets orally, (group B): 50 patients received sildenafil citrate 25 mg at eight hours interval, (group C): 50 patients received nifedipine tablets 20 mg orally followed by 10-20 mg three to four times daily in combination with oral administration of sildenafil citrate 25 mg at eight hours interval.
Results; Uterine contraction were significantly decreased in the group that received both nifedipene& sildenafil medications than those who received nifedipine only and sildenafil only.
Conclusion; Using the evidence-based practices, our study offers hope that the combination of sildenafil citrate with nifedipine is superior to nifedipine alone or sildenafil alone in preventing threatened preterm labor. The use of sildenafil in addition to nifedipine prolong the period of tocolysis delayed the onset of Preterm labour and reduces neonatal intensive care unit (NICU) admission.
