Abstract

The clavicle is a long bone that serves as a strut between the shoulder blade and the sternum and fracture clavicle is a common traumatic injury around shoulder girdle. About 70–80% of these fractures are in the middle third of the bone and less often in the lateral third (12–15%) and medial third (5–8%).Direct fall on shoulder is most common mechanism of injury of fracture clavicle other mechanism include direct trauma to clavicle and fall on outstretched hand.Allman classification of fracture clavicle is first widely accepted classification system although it is not helpful in describe displacement, comminution, or shortening, all potentially important prognostic and treatment variables. Recently Robinson classification system described in 1998 has prognostic and therapeutic value.The ideal management of the fracture of the mid-shaft of the clavicle remains a topic of debate .most cases are treated conservatively. immobilization by arm sling or figure of eight or both together and give result comparable to operative treatment however patients with displaced clavicle fracture treated with conservative treatment are unhappy with significant residual strength due to high rate of mal-union and shortening, and this can have a significant impairment in shoulder functions of an active young adult associated with shoulder weakness, easy fatigability symptoms, winging of scapula, thoracic outlet syndrome and droopy asymmetric shoulder.According to extracted data operative treatment is superior to non-operative treatment in the management of displaced mid shaft clavicle fractures, General agreement exists on that primary surgical management of clavicular fracture is advised for special indications only.
