Abstract

To study the role of the doppler velocimetry and indices of the umbilical, middle cerebral, uterine arteries in prediction of adverse neonatal outcome in post-term pregnancies. A prospective cross-sectional study at Cairo university hospital for obstetrics and gynecology conducted on 50 pregnant women of prolonged pregnancy whose gestational age 41 or more weeks. middle cerebral artery (MCA), umbilical artery (UA) and uterine arteries (UtA) doppler and MCA - PI / UA - PI ratio, MCA - RI / UA - RI ratio were assessed. They were divided into two groups based on the presence or absence of adverse neonatal outcome. 32% had adverse neonatal outcome as meconium aspiration syndrome (12%), the ratio of middle cerebral artery to umbilical artery resistivity index {u2264}1.1 can be used as a sign of adverse neonatal outcomes in prolonged pregnancies with a sensitivity of 81.3% and a specificity of 97.1%, and the ratio of middle cerebral artery to umbilical artery pulsitility index {u2264}1.9 can also be used, the mean value of the umbilical artery resistivity index was significantly higher in the group with adverse neonatal outcome than the normal group
