Abstract

Percutaneous collagen induction (also known as microneedling) is a minimally invasive procedure in which microneedles are used to create thousands of microscopic wounds in the dermis after penetrating the epidermis. These wounds trigger the wound healing process, resulting in the release of several growth factors that stimulate the migration of fibroblasts and new collagen formation, making microneedling useful for treating a wide variety of skin diseases and scars caused by acne.
A variety of devices, including tattoo guns, dermastamps, dermarollers, dermapens, and others, may be used for miconeedling, which is widely regarded as a simple, straightforward, safe, and inexpensive technique of inducing collagen.
When using a dermaroller, a kind of microneedling tool with 192 needles measuring between 0.25 to 3 millimetres in length and 0.07 millimetres in diameter, the resulting wound density is 250 holes per square centimetre up to the papillary dermis. It is applied directly to the skin in a variety of directions (vertically, horizontally, and diagonally).
The Dermapen is a pen-shaped automated microneedling device. This instrument utilises single-use needles and guides to provide for individualised needle length adjustments during fractional mechanical resurfacing procedures. Needles numbering from 9 to 36 are stacked in rows at the tip. It uses a rechargeable battery to run at two different vibrational stamp speeds (700 and 412 cycles per minute, respectively) on the same charge.
The purpose of this research was to evaluate the efficacy of dermapen (vertical microneedling) and dermaroller (horizontal microneedling) in the treatment of atrophic acne scars and fine lines and wrinkles on the face.
Twenty participants were included in the research, ten each in group I (post-acne scars) and group II (facial ageing). Microneedling with a dermaroller (DR) on the right side of the face and microneedling with a dermapen (DP) on the left side of the face, 6 times, 2 weeks apart, with a 2.5 mm needle length for group I (acne scars) and a 1 mm needle length for group II, was used for all instances (facial aging).
At 2 weeks post-treatment, all patients in the acne scars group showed clinical improvement on both sides of the face, with a slight better non-significant improvement on the right side (DR). In the facial ageing group, there was no statistically significant difference between the two sides of the face.
There was no statistically significant difference in mean epidermal thickness between the two sides of the face in the acne scar and facial ageing groups, but there was a substantial rise in epidermal thickness on both sides of the face.
Regarding collagen bundles, this research found that two weeks after treatment, collagen bundle deposition increased in both groups, with the deposited collagen being finer and more structured than the baseline, with smaller interfibrillar gaps. Meanwhile, new elastic fibres grew on both sides of the face in the acne scar and facial ageing groups at 2 weeks following the last session, with no discernible difference between the two sides.
Finally, microneedling has shown to be a simple, secure, and efficient procedure for skin remodelling. There was no statistically significant difference between the use of horizontal microneedling (dermaroller) and the use of vertical microneedling in the treatment of atrophic post-acne scars and face ageing (dermapen). More research are needed in the future with a bigger sample size and longer follow-up duration to corroborate such results.
