Abstract

Seroma is one of the most prevelant bothersome complications after breast surgeries and can lead to delay patient recovery and post-operative adjuvant therapy for cancer.
Many factors affect seroma, recently one of them is the type of drain, It was reported that the new Jackson-pratt drain offer advantage over the conventional redivac one.
The aim of this study was the evaluation the effect of using the Jackson-pratt drain and compare the effect of using the Jackson-Pratt drain and Redivac drain in patients undergoing oncoplastic surgeries in the prevention of seroma formation.
The Clinical Human Research included:
80 patients who had undergone oncoplastic breast surgeries for breast cancer. Patients were randomized to either group A and group B by a double-blinded randomized controlled method. (double blind envelop method), group A (using Jackson-Pratt drain) and group B (using conventional Redivac drain). They were admitted to the surgical unit of the Medical Research Institute, Alexandria University, in the period from November 2021 to December 2022. All cases were followed up and evaluated for up to one year.
Full laboratory and radiological investigations were performed on all patients.
Surgical technique:
4 different surgical techniques were performed:
 Conventional conservative breast surgery (20 cases)
 Oncoplastic conservative breast surgery (20 cases)
 Modified radical mastectomy (20 cases)
 Mastectomy with Reconstructive breast surgery by a flap (20 cases)

The findings can be summarized into:
Age: the mean age of the included cases was 47.2 and 48.2 years in the Jackson Pratt drain and redivac groups, respectively.
Body mass index (BMI): The BMI had mean values of 31.0 and 30.0 kg/m2 in the Jackson Pratt drain and redivac groups, respectively.
Systemic comorbidities: Diabetes mellitus was present in 7 (17.5%) and 6 (15%) of cases.
Breast size: There was no statistically significant difference between the two studied groups.
Time until drain removal: The range in group A Jackson- pratt drain was 7-16 days (mean 11.5 ) and 4-18 days ( mean 10.8) in group B Redivac drain
Total drain discharge: In the Jackson drain group (685.33 ml) vs. (915.27 ml) in the redivac drain group.
Seroma incidence: The number of cases in group A (Jackson drain) was 5 (12.5%) and 15 (37.5%) in group B (redivac drain).
Amount of seroma aspirated: 190-440 ml (mean 278 ml) in group A while seroma aspirated 100-810 ml (mean 484.5 ml) in group B.
The number of Clinic visits: for seroma aspiration was in the range of 2-4 times (mean 2) in group A and in group, B was 2-7 times (mean 4.4).
Incidence of a clogged drain: Jackson drain group showed a significant decrease in the incidence of clogged drains 2.5% to 20% in the redivac drain.
The number of cases complicated with delayed seroma: was 1 case (2.5%) in group A and 4 cases (10%) in group B.
Pain scores: The Jackson drain group expressed significantly lower pain scores compared with the redivac drain group (P < 0.05).
