Abstract

Vitiligo is a multifactorial disease. In general, it’s agreed that there’s a lack of functional melanocytes in vitiliginous skin and that the loss of histochemically determined melanocytes is the cause of damage. CD8+ T cells are the principal cells that account for the direct destruction of melanocytes.
Chemokines have a pivotal role in the regulation of homing of immune cells as CXCL10 that’s essential for the progression and maintaining of depigmentation in vitiligo.
Regarding the management of vitiligo, NB-UVB is indicated for generalized NSV in particular the active spreading types. In conjunction with NB-UVB, simvastatin was proved to be an efficient treatment agent for vitiligo. It can inhibit chemokine synthesis by endothelium, block chemokine receptors on T- cells and might have an important therapeutic role for vitiligo.
Thus, the current observational study aimed at studying the effect of NB-UVB phototherapy versus NB-UVB combined with simvastatin on the plasma levels of CXCL10 & CCL8 in vitiliginous patients.
The study was conducted on 50 adult cases diagnosed with vitiligo and divided into 2 groups:
• group A included 25 vitiliginous cases that were treated using NB-UVB alone for 3 months.
• group B included another 25 cases that were treated using NB-UVB in addition to simvastatin 40 mg / day for successive 3 months.
To establish the diagnosis, disease extent & severity, VASI and VETF were assessed for all cases before and following therapy . The plasma levels of CXCL10 & CCL8 were assessed for all cases before and following treatment via ELISA kit.
It was proved that in comparison to baseline there was statistically significant reduction in the plasma levels of CXCL10 & CCL8 in group B while the reduction in group A was non-significant. On the other hand, decreased VASI score was significant in the two groups. As a conclusion, simvastatin has additive role to NB-UVB in treating vitiligo.
