Abstract

Background; There were numerous previous studies comparing open appendectomy (OA) with laparoscopic appendectomy (LA). Although most of these have concluded that the LA is good as OA, there has been considerable controversy as to whether LA is superior to OA or not, Aim and objectives; our primary objective was to compare the laparoscopic approach and the open technique in the treatment of chronic appendicitis in the Departement of General Surgery at Assiut University hospitals and assess the short term follow up of patients undergoing this procedure regarding to pain, bleeding, recovery time, hospital stay, infection, and cosmetic appearance, Subjects and methods: In this prospective randomized study, Eighty-six patients were assessed for eligibility, 25 patients did not meet the criteria and 11 refused to participate in the study. The remaining 50 patients with chronic appendicitis were randomly allocated into two groups (25 patients each). All patients were followed-up and analyzed statistically, Result; Operative time was significantly longer in Laparoscopic Appendectomy group compared to open Appendectomy group (P= 0.005), Laparoscopic Appendectomy was associated with significantly shorter hospital stay with a median duration of one day as compared to open appendectomy with a median of two days (P<0.001) and The severity of postoperative pain was significantly lower in patients subjected to Laparoscopic Appendectomy (group II) when compared to those subjected to open appendectomy (P<0.001) with no statistically significant difference in terms of complications, Conclusion; In conclusion, the laparoscopic approach is a safe and efficient operative procedure in chronic appendectomy and it provides clinically beneficial advantages over open method (including less recovery time, shorter hospital stay, less pain, decreased need for postoperative analgesia).
