Abstract

Treatment of distal dia-metaphyseal tibial fractures with or without articular extension is challenging because of their unique anatomical characteristics of subcutaneous location with precarious blood supply and proximity to the ankle joint.In our study, that was done on 20 closed distal tibial fractures with mean age (49.5 years).Modes of trauma were RTA 70 % and FFH 30 %.We operated all patients by using MIPO, the operative time average was (60-90 min) we fixed the fractures through 5 cm distal incision over the medial malleolus and proximal about 2-3 cm at the end of the plate.Subcutaneous tunnel over the periosteum was created between two incisions by blunt dissection; we inserted the plate through this tunnel and fixed it under fluoroscopy.Patients were followed up clinically and radiologically for at least 6 months post operatively.
Clinical results graded as excellent (70 %), good (25 %), fair (5 %).
Union occurred in 19 patients (95 %).We reported complications such as one patient with superficial skin infection who is diabetic patient and one had non-union of the fracture.We didn’t report any cases of leg discrepancy or any cases of osteoarthritis till now.In summary, with ideal reduction and maintenance, plate fixation is a recommended method for extra-articular fractures of distal tibia unless there is severe injury of the soft tissue.
