Abstract

Background: Patients with facial burns are real challenge for the clinician as not only the functional but also the aesthetic needs as well. Although there are variable options of topical treatment, the cornerstone of management, there is no gold standard agent.
Objective: To improve the aesthetic and the functional outcome of facial burn.
Methods: This concurrent self-controlled open label clinical trial was done on 15 patients suffering from facial burn attending the burn unit of Suez Canal University Hospital.
Facial burns of each patient were divided in two equal parts: part A treated with local hirudin (Thrombex cream), and part B treated with topical beta-sitosterols (MEBO or Pentaburn ointment) which was considered as the gold standard and thus control being the most commonly used in our region.
In both groups, the local cream was used as exposed dressing in combination with cold saline fomentation and gentle daily cleansing without exposure to heat or sun light with good nutrition, plenty of fluids and rest. General and standardized care was done to both groups.
Daily follow up of the patients at early stage until complete healing was done by visual assessment of healing rate by serial photographs of the wounds. In addition, subjective assessment was done using patient and observer scar assessment scale (POSAS) to assess healing by both patients and observer scales. Pain was assessed using Visual analogue scale (VAS).
