Abstract

The aim of the study was to compare between eight plate and staples in the management of angular knee deformities in children older than 3 years after 12 months of follow up in order to reduce morbidities resulting from angular knee deformities in children, in addition to improving their quality of life.
Twenty patients ’including 16 bilateral and 4 unilateral, total 36 angular deformed knees with open physis’ were included after being admitted to Suez Canal University Hospital, Ismailia, Egypt. The study participants were assigned in two groups
group A: deformed knees had been treated with eight plate and
group B:deformed knees had been treated with staples.
The mean age of the patients was 5.30 ± 2.80 years with a range of age between 3.5 to 13 years. Males formed 52.8 % of the patients in the study while females formed 47.2 %. Regarding type of deformity, most of the patients had genuvarum (83.3%) . About94.4% of group A had genuvarum deformity, while 72.2% of group B were genuvarum.
The patients were clinically and radiologically examined to assess their functional abilities. X-ray both lower limbs standing long film including hip, knee and ankle were reviewed to measure the mechanical tibio-femoral angle ’mTFA’, mechanical lateral distal femoral angle ’mLDFA’, and mechanical medial proximal tibial angle ’mMPTA’ for all patients.
There was no statistically significant difference between the two groups in regard to the mechanical lateral distal femoral angle (mLDFA) and the mechanical medial tibio-femoral angle (mTFA) either before operation and on 12 month post-operative follow up. Also, There was no statistically significant difference between the two groups in regard to the mechanical medial proximal tibial angle (mMPTA) before operation.
The rate of correction of mechanical medial proximal tibial angle( mMPTA) with eight plates(3.21 ± 11.74) is significantly higher compared to rate correction with staples 12 month post-operative (11.77 ± 4.09) degree. Regarding the complications, one patient in staple group had failure of proximal tibial staple after 6 months. Meanwhile, one patient in 8-plate group had wound infection.
By the end of the follow up, there was a significant correlation between the rate of correction of mechanical medial proximal tibial angle( mMPTA) between the 2 groups.On the other hand, there was an insignificant correalation between the rate of complications between the 2 groups.
