Abstract

The transrectal (TR) prostate biopsy is the most common technique to diagnose prostate cancer in most countries. Transperineal (TP) prostate biopsy, on the other hand, has been gaining popularity in several centers worldwide due to its lower rates of infectious complications with a comparable CDR in both techniques.Therefore, we aimed in the present study to comprehensively evaluate and to compare the outcome of (TRPBX) and (TPPBX) in the diagnosis of PCa with regards to; procedural time, complications, CDR, safety and feasibility of both approaches.
200 patients with a suspicion of PCa collected between August 2021 to August 2022 were randomly divided into two groups: group A (Included 100 patients who underwent (TPPBx) approach) and group B (Included 100 patients who underwent (TRPBx) approach). Patients with abnormal elevated total PSA and/or abnormal free/total PSA or PSA density and/or suspicious digital rectal examination findings and/or hypoechoic nodules by TRUS and/or suspicious lesion on prostate mpMRI were included in this study. While patients with untreated urinary tract infection and patients with uncorrectable coagulopathy were excluded.
The results were summarized as following:
There were no significant differences regarding patients’ age, BMI, prostate volume, total PSA, PSA density of the patients in both groups.
Considering the procedural time: There was significant difference in the procedural time between the two groups (mean procedural time in group A, 18.68 ± 0.96 and in group B, 12.38 ± 1.12) (P <0.001).
