Abstract
The COVID-19 pandemic, caused by the SARS-CoV-2 virus is an emergent and highly contagious disease that has had a profound impact globally, leading to millions of confirmed cases and deaths. COVID-19 has several clinical spectra of infection encompassing a symptomatic infection, mild upper respiratory tract illness, and severe viral pneumonia with respiratory failure and even death, with many patients being hospitalized with pneumonia. The whole world faced new waves of COVID-19, forcing laboratories to the maximum of their testing capacity despite limited reagent supplies. RT-PCR remained the most rapid and sensitive diagnostic method for COVID-19. Cytokine storm is an interesting point in SARS-CoV-2 infection. Elevated levels of inflammatory cytokines IL-6, TNF-α, IL-1β and IL-2 and chemokines (IL-8) were observed in COVID-19 patients with more severe disease. The host characteristics such as gender, concomitant comorbidities, ethnicity, and age group are ultimately related to the observed variation in clinical presentation and consequences of SARS-CoV-2 infection.

In this study, the cohort of 900 enrolled cases was divided into two groups: group I (Wave 3 Patients; n=380), consisting of 380 patients collected during the third wave of the SARS-CoV-2 pandemic from June 1st to the end of August 2021, and group II (Wave 4 Patients; n=520), comprising 520 patients gathered during the fourth pandemic wave from September 1st to the end of November 2021. The present study delved into IL-6 as a marker for disease progression due to its pivotal role in the cytokine storm associated with COVID-19, highlighting its significance as a therapeutic target. Our data revealed a notable correlation between elevated serum IL-6 levels and the severity of illness, particularly in severe and critical cases. Increased IL-6 levels were closely linked to adverse clinical outcomes, including disease advancement, severe complications, and heightened mortality rates, emphasizing the criticality of monitoring this cytokine in COVID-19 patients. Our findings indicated that the initial IL-6 serum levels could predict the severity and mortality of COVID-19, with cut-off values of more than 17 pg/ml and 23 pg/ml, respectively. The association between heightened IL-6 levels and various clinical and laboratory markers of systemic inflammatory response, such as CRP, ferritin, D-dimer, and LDH, further validated IL-6 as a dependable marker for assessing the inflammatory status in COVID-19 cases.
