Abstract

Summary
Chemotherapy works by stopping or slowing the growth of cancer cells, which grow and divide quickly, and it is used to treat or ease cancer symptoms. An important responsibility of oncology nurses involved in the preparation, administration of antineoplastic drugs (all routes), safe drug handling and disposal, management of untoward reactions. previous studies have reported that extent and type of poor safe handling of chemotherapy occurring in oncology patient care settings which leads to significant morbidity and cost, and the majority of these cases are preventable. So, training and educating oncology nurses requires innovative approaches that overcome some of the difficulties seen with traditional learning programs by allowing flexibility in time, place, and pace as the online programs to deliver education based on the best available evidence or best practice, knowledge and best attitude for improving safety handling of chemotherapy.
Aim of the study:
This study aimed to evaluate the effect of online educational program versus traditional educational program on oncology nurses’ performance and their satisfaction through the following:
• Assessing of nurses’ knowledge, practice, attitude and their satisfaction.
• Planning and implementing online educational program versus traditional educational program for nurses’ performance.
• Evaluating online educational program versus traditional educational program on oncology nurses’ performance and their satisfaction
Research Hypothesis:
The current study hypothesized that:
1. The mean of total knowledge score of oncology nurses who are exposed to online program will be higher than oncology nurses who are exposed to traditional program.
2. The mean of total practice score of oncology nurses who are exposed to online program will be higher than oncology nurses who are exposed to traditional program.
3. The mean of attitude score of oncology nurses who are exposed to online program will be higher than oncology nurses who are exposed to traditional program.
4. The mean of total satisfaction score of oncology nurses who are exposed to online program will be higher than oncology nurses who are exposed to traditional program.
Research design:
A quasi-experimental design was utilized to achieve the aim of the study.
Setting:
The study was conducted at the inpatient and outpatient oncology Center, BMT unit, fifth and sixth medical oncology departments at third floor that affiliated to Ain Shams University Hospital, Cairo, Egypt.
Subjects:
A convenient sample of all available oncology nurses included 80 nurses at the previously mentioned settings that affiliated to Ain Shams University Hospital were divided into two groups (traditional program group and online program group).
Study tools:
The tools used for this study were:
I- Assessment of self-administered Questionnaire sheet
An assessment questionnaire sheet was developed by the researcher to assess oncology nurses’ knowledge in caring that provided for oncology patients with CDs. It was included two parts:
Part 1: It was used to assess demographic data of studied oncology nurses.
Part 2: It includes five sections:
• Oncology nurses’ knowledge regarding CDs.
• Oncology nurses’ knowledge regarding PPE.
• Oncology nurses’ knowledge regarding CDs preparation.
• Oncology nurses’ knowledge regarding CDs administration.
• Oncology nurses’ knowledge regarding CDs waste management.
II-Nurses’ Observational Checklist:
It was developed by the researcher based on recent literature review to assess oncology nurses’ practice level regarding caring of oncology patients undergoing CDs, and it was used pre-and post-implementation of the education program. It composed of 4 following procedures:
• Safe Preparation of Cancer Chemotherapy / Cytotoxic Drugs Checklist.
• Safe Handling & Disposal of Hazardous Drugs Checklist.
• Intravenous IV Administration of Cancer Chemotherapy / Cytotoxic Drugs via Intermittent Piggyback Infusion in a Peripheral Vein Checklist.
• Intravenous IV Administration of Cancer Chemotherapy / Cytotoxic Drugs via CVAD checklist.
• IV Direct Administration of Cancer Chemotherapy/Cytotoxic Drugs via Peripheral Vein Checklist.
III- Nurses’ attitude questionnaire:
It was developed by the researcher based on recent literature review. It was used to assess nurses’ attitudes towards the importance of safe handling of CDs.
IV-Nurses’ satisfaction questionnaire:
This questionnaire developed by the researcher after he had reviewed the related literatures. To assess the overall nurses’ satisfaction regarding the traditional face-to-face education program and online education program. It consisted of four sections and 24 items:
A. Scientific content of education program.
• Educational environment.
• Researcher (trainer).
• Overall satisfaction.
Results of this study showed that:
The results revealed that, there were highly statistically significant differences between the traditional group and online group regarding post and follow up of the total level of knowledge about the safe handling of chemotherapy (p<0.001), in which the mean score of post and follow up tests was 34.3±5.39and 30.4±3.73 for oncology nurses in traditional group, while it was 45.2±3.75 and 38.7±3.57for oncology nurses in online group respectively.
The results displayed that, there were highly statistically significant differences between the two studied groups regarding post and follow up of the level of skills about the practice of safe handling of chemotherapy during preparation & administration (p<0.001), in which the total mean score of post and follow up tests was 46.3±5.06 and 38.5±4.72 for oncology nurses in traditional group, while it was 52.9±4.79 and 47±4.47 for oncology nurses in online group respectively.
The results showed that, there were highly statistically significant differences between the two studied groups regarding post and follow up of the level of skills about the practice of safe handling & Disposal of chemotherapy (p<0.001), in which the total mean score of post and follow up tests was 40.4±4.07 and 33.6±3.75for oncology nurses in traditional group, while it was 47.8±3.18and 40.91±2.92for oncology nurses in online group respectively.
In relation to demographic data, there has no statistically significant relationship with oncology nurses in the traditional and online groups as regards total level of knowledge and practice scores before, post-program and follow up of program implementation.
There was highly statistically significant positive correlation between total satisfaction and online education program post program implementation. While, there was only statistically significant positive correlation between total satisfaction and traditional face to face education program post program implementation.
The study concluded that:
Implementation of the online and traditional educational program had positive effect on improving oncology nurses’ performance and their satisfaction. Concerning the implementation of online educational program method, there had more positive effect on improving the oncology nurses’ knowledge and practice than traditional program method, and there was highly statistically significant difference between the two studied methods, but there were no statistically significant difference between two studied methods traditional and online educational program regarding the oncology nurses’ attitudes about the importance of chemotherapy safe handling. Concerning the total online education satisfaction post program implementation was highly statistically significant, but the total traditional face to face education satisfaction post program implementation was only statistically significant.
The current study recommended that:
• Periodically online education program should be conducted for all oncology nurses who are handling CDs to improve knowledge and practice of safe handling measures and to reduce the harmful effects of these drugs among oncology nurses is considered of great importance.
• Oncology nurses should be encouraged to attend online scientific meetings and training program to gain updated skills and knowledge about safe handling of CDs for proper nursing services.
• Increase oncology nurses’ awareness about policies and procedures guidelines related to CDs safe handling.
• Development of hard and soft copy of manual procedure regarding safe handling of CDs that should be available in each oncology word and on the internet hospital site as a reference, and should be up to date periodically to suite new trends of safe handling CDs.
• Further researches are recommended to assess the factors that affecting on the online education programs implementation about safe handling and hazards prevention of CDs.
• Replication of the present study on a large sample representing different hospital settings in Egypt in order to generalize the results.
