Abstract 13 systemic lupus erythematosus (SLE) patients (26 eyes) with no clinical ophthalmic manifestations, diagnosed according to the European League Against Rheumatism/American College of Rheumatology Classification Criteria for Systemic Lupus Erythematosus and 13 healthy subjects (26 eyes) were enrolled. Patients and controls were matched regarding age and sex. Optical coherence tomography equipped with Angiovue (OCTA) was used to obtain three-dimensional retinal thickness maps and microvascular images of the superficial and deep retinal capillary plexuses around the macula. SLE disease activity was assessed by the SLEDAI-2 K scoring system and patients were classified into two subgroups: low disease activity (LDA) (SLEDAI≤6) and high disease activity (HDA) (SLEDAI>6). Results: Patients with SLE, even in the absence of ophthalmic manifestations, presented with a significant reduction in retinal thickness of all the macular sectors (P<0.05) with a highly significant reduction in the parafoveal and perifoveal temporal and superior regions and parafoveal inferior region (P< 0.01). A statistically significant reduction in superficial capillary plexus (SCP) and DCP vessel density (VD) between patients and controls was found in the whole image area, fovea and temporal parafoveal region (P<0.05). No statistically significant difference (p>0.05) in foveal avascular zone (FAZ) parameters was found except for foveal density (FD) was found to be significantly lower (P<0.05) in SLE patients than in healthy controls. There was no statistically significant difference between SLE groups with LDA and HDA in FAZ or any of the other areas, including SCP and DCP. However, there was a statistically significant difference between HDA and control groups concerning SCP VD of the whole image region and DCP VD in the parafoveal temporal region (P<0.05). Correlation between OCTA findings and SLEDAI-2 k revealed a negative correlation with retinal thickness in the inferior region of the perifovea (P=0.05), while no significant correlation was found between SLEDAI-2 k and the retinal vessel density in either layer. Additional statistically significant negative correlations between SCP and DCP VD and age, disease duration, CRP, Serum creatinine and liver enzymes were found. Conclusion: Patients with SLE had diminished retinal thickness and reduced superficial and deep capillary plexus densities compared to healthy controls. OCTA findings (SCP VD and DCP VD) in SLE patients significantly correlated with disease duration and laboratory data (acute phase reactants), while retinal thickness only, correlated with disease activity (SLEDAI-2K score). Retinal involvement in systemic lupus erythematosus may remain asymptomatic for many years and may finally be associated with severe visual deterioration, therefore it is essential for patients with SLE to be screened by OCTA even when asymptomatic and any OCTA vascular changes should prompt a closer follow up.
