Abstract Case control study was done for assessment of troponin I level of patients with AGA to detect if there is a role of troponin I as a cardiac biomarker for cardiac affection in the disease (AGA) pathogenesis and its associations and looking for a hidden cardiac problem. The study was conducted on 75 participants; 50 patients suffering from Androgenetic alopecia, in addition to 25 control normal healthy persons (male and female), age and sex matched individuals as a control group. The AGA group mean age was 34.4 years, they were 31 males (62%) and 19 females (38%). In addition to 25 healthy control group, they were 11 males (44%) and 14 females (56%). there was no significant difference between both groups in smoking. AGA group had significantly lower absolute value of GLS, higher troponin I when compared to control group (p<0.001). Troponin had better AUC than GLS which had better AUV than EF (AUC=0.920, 0.832, 0.696 respectively). Also, troponin I showed significant positive correlation with age, GLS, grades, as well as significant negative correlation with EF.
