Abstract

Bipolar electrodesiccation is an effective, rapid, cheap and relatively safe haemostatic technique for almost alllaparoscopic procedures. It was effective in securing large pedicles such as the infundibulo-pelvic ligament and uterine vessels. Bipolar coagulation is also useful for ablation of endometriosis, and for ”bloodless” adhesiolysis and opening the peritoneal pouches. Bipolar electrodesiccation is of comparable speed to staples and significantly more rapid than sutures in most of the procedures. It lacks the additional costs that is required for other techniques apart from those required for maintenance of generators and forceps. The economical aspects of laparoscopy, in our country and other parts of the world where health funding is limited, is of considerable priority. Bipolar coagulators have the versatility to be used in various situations especially in emergency conditions and have the capacity to cope with the developing challenges of endoscopic surgery. We have not had major complications from electrosurgery, namely, burns of the bowel or the ureters. Judicious use of electrosurgery in the hands of knowledgeable surgeon is safe. The discomfort experienced after bipolar surgery needs further investigation to see if this is an inherent disadvantage of the technique. Neither the observational or randomised controlled studies were able to confirm that the use of the linear stapler in securing the infundibulo-pelvic ligaments and uterine
