Abstract

Perimetry is the most accepted routine investigation evaluating the efficacy of ocular therapy in glaucomatous patients. Twenty seven eyes of 23 patients suffering from primary adult glaucoma subjected either to laser were trabeculoplasty or trabeculectomy. After full ocular examination. Pre-and post operative fields of all eyes were measured employing Goldmann perimeter. A monthly follow UP measurment of visual fields continued for one year. Definite regression of the visual fields. in the form of widening of isopters of the seeing area and or disappearance of certain scotoma. were observed in 6 out of 27 eyes (22%). Improvement of visual field was observed in young patients whose glaucoma was diagnosed early and who have had a C/D ratio not more than 0.5 and pre-operative tension around 25 m mHg without medication. Stablization of the visual fields was observed in 13 eyes (49%) of patients whose glaucoma was diagnosed withen duration less than 3 years and who have had a C/D ratio not mere than 0.7 and pre-operative tension around 30 rom Hg. Mean-while in 8 eyes (29%) inspite of control of ocular tension. the visual field showed aefinite deterioration in the form of constriction of the isopters, increase in extend of existing scotoma, appearance of fresh scotomas and or diminution of retinal sensetivity bordering scotomas, five patients of this group were old aged patient whose glaucoma was diagnosed late, and have had C/O more than 0.7 with pale neural rim tissue and pre-operative tension around 34 mm Hg, in the remaining 3 patients inspite of early diagnosis of glaucoma and small sized glaucomatous cupping, the visual field continue to deterio - rate due to devolopment of post operative complications ”including lens opacity in 2 cases and progressive vitreal haze in a third case. In conclusion, the present study reveals that post- operative improvement of the visual fields is likely to occur in patients under the age of 45 years whose glaucoma is diagnosed early with pre-operative ocular tension around 25 rom Hg and C/O ratio less than 0.5 with healthy rim tissue. Risk factors such as hypertension, diabetes mellitus, areterio- sclerosis seems to playa definite role in the deterioration of the visual field in chronic glaucoma, this may be an added evidence confirming that a reversible functional disturbance occurs much earlier than permanent pathological damage. Hence, early diagnosis and correct timing of glaucoma surgery is of paramount importance .
