Abstract

Glaucoma filtration surgery traditionally uses a limbus-based conjunctival flap. Trabeculectomy with addition of a lamellar scleral flap provides resistance to aqueous humour flow through the partial thickness corneoscleral opening into the anterior chamber and directs the flow of aqueous posteriorly away from the limbus. This permits the opportunity to place the conjunctival incision anteriorly at the limbus (fornix-based), away from the area of filtration. The aim of this study is to compare between fornix-based and limbal¬based conjunctival flap techniques with subscleral trabeculectomy for , primary open angle glaucoma as regards the effect on lOP control, depth of the anterior chamber, appearance of bleb and incidence of complications with each technique. Fifty eyes with bilateral primary open angle glaucoma were included in this study. A fornix-based flap (group A) was fashioned in one eye and a limbal based flap (group B) in the fellow eye. In this study, the postoperative lOP control was 100°,10 and 960/0 of eyes treated by trabeculectomy with limbus based and fornix-based c0tiUllctival flap procedures respectively. The mean preoperative i" ~ "aocular pressure was 38.44 mmHg and 38.10 mmHg in group A and group B respectively.
