Abstract

It is well established that the dioptric power of an eye depends on corneal curvature, crystalline lens position and power, and axial length. In any group of emmetropic eyes there will be found a wide range of these four variables, despite their identical overall refraction. This is why spectacle refraction alone gives such a poor guide to implant power. Although preoperative calculation of 10L power will guard against the p( stoperatr.e refractive surprises, yet there are a number of sources of error 111 the prediction of postoperative refraction after 10L implantation as error of axial length determination or keratometric measurements and the use of predicted postoperative anterior chamber depth in theoretical formulas. The rOL formula may itself be the source of error. The theoretical and empirical formulas give good results. It is important that the surgeon determines which equation is more accurate for his patient, especially in high myopes and hyperopes since the rOL formulas differ most significantly in those groups of patients.
