Abstract

Chronic impingement syndrome is a very common painful condition affecting the shoulder. Many pathological lesions are encountered in the diagnosis. Clinical examination and plain X-Ray films can not define the exact pathology of impingement. It is important to make a specific pathological diagnosis to outline the line of treatment. Many diagnostic modalities had been used to varify the diagnosis including computed tomography, bursogrpahy, arthrography, ultrasonography. and magnetic resonance imaging with variable degrees of sensitivity and specificity. The aim of this work is to evaluate the exact cause of impingement syndrome detected during surgery and therefore evaluate the sensitivity and specificity of magnetic resonance imaging in diagnosis. We have reviewed the literature with regard to anatomy, biomechanics, pathology, diagnosis, and treatment of chronic impingement syndrome and rotator cuff tears with special attention to MRI (being the material of investigation). The material of this study included twenty patients aged 40-60 years old with chronic impingement syndrome not responding to non operative treatment for at least 6 weeks. All cases have been subjected to through clinical examination, plain X -rays, MRI, and surgery.
