Abstract

This work was conducted to study the effect of capto¬pril on the hemodynamically significant VSD in infants and children. The study included 18 infants and child having signif¬ica~t VSD. They were subjected to detailed history taking, through cl in ical and echocard i ographic Qp/Qs rat i o , Capto¬pril was then given to the patients, the dose was 1 mg/kg/day in two divided doses. We follow up the patients every 2 weeks for one month, where the patients were sub¬j ected to frequent cl i nica I check i ng and echocard 1 ograph ic evaluation. After 6 months, the patients were checked clini¬cally by measuring heart rate, respiratory rate and gain of weight. The results obtained showed a marked improvement clinically as regard manifestations of heart failure-’ where there was significant (P < 0.05) decrease in blood pressure without subjective complain of hypotension. As regard echo¬cardiographic evaluation, there was significant improvement of myocardial function (P < 0.05), significant reduction in the size of the defect (P < 0.05), significant reduction of Qp/Qs ratio (P < 0.05), highly significant reduction of end systolic left ventricular pressure (ESPLV) (P < 0.001), significant decrease of mean pulmonary artery pressure (MPAP) (P < 0.05), and highly significant reduction of end systolic right ventricular pressure (ESPRV) (P < 0.001).
