Abstract Thirty patients underwent modified Heller’s myotomy for treatment of achalasia in the Gastroenterology center1978 and 1991. They were evaluated clinically,endoscopically,adiologically,manometric and by’radionuclide study. The overall success rate was 80%. The incidence of reflux was 16.6%. Revision myotomy was done for 2 cases through transthoracic approach and early postoperative evaluation has revealed satisfactory results.Esophageal manometry and radionuclide esophageal transit were excellent tools in assessing esophageal function and gave us a peculiar pattern in both complete or incomplete myotomy. Although modified Heller’s myotomy is accepted as the surgical procedure of choice for treatment ~fachalasia, yet the 16.6 % incidence of postoperative reflux in this thesis raised the question of adding antireflux procedure. |
