Abstract

Orthotopic bladder substitution has gained wide acceptance after radical cystectomy in the last decade. Different bowel segments with different configurations have been utilized to construct intestinal neobladders. However, it is universally accepted that detubularization and reconfiguration provides the best urodynamic features of the reservoir. In spite of this fact, 20 to 30% of patients suffers from nocturnal enuresis. Thirty continent patients (15 Kock and 15 ”W” neobladders) and thirty enuretic patients (17 Kock and 13 ”W” neobladders) were evaluated by urine cultures, ascending pouchography, micturating cystourethrography and urodynamic studies using uroflowmethy, filling cystometry and urethral pressure profilometry. ” .Positive urine cultures were frequent among enuretic than continent patients with statistical significance among the ”W” neobladder group. Mean residual urine volumes were 50.67 and 42.67 ml in continent Kock and ”W” neobladders respectively while those of enuretic cases were 97.65 and 162.31 ml in Kock and ”W” neobladders respectively. This difference between continent and enuretic cases was statically significant in the ”W” neobladder group. The Flowmetric characters were comparable in both continent and enuretic groups of patients with mean maximum flow rate above 15 ml/sec. in all groups and slightly below 15 ml/sec. in the enuretic group of ”W” neobladder (mean= 13.85 :t 7.06 ml/sec.). Maximum cystometric capacity was comparable in both groups of patients of both types of reservoirs with no statistically significant difference. On the other hand pressure characters were significantly higher in the enuretic than in the continent patients for both types of reservoirs. Pressure at ” " ~ " ” maximum capacity ranged from 12 to 54 cm water among the enuretics while it was from 7 to 35 cm water among the continent cases. Maximum amplitude, frequency and duration of uninhibited segmental contrations were also higher in enuretic than in the continent cases.
