Abstract

A retrospective study was carried out on 27 children with 49 idiopathic clubfeet treated by Turco’s posteromedial release at the Orthopaedic Department of Mansoura University Hospital. Evaluations included review of hospital records, history of treatment prior to surgery, clinical examination, and radiographic examination. Assessment of the results was made according to a modification of the scoring system described by Lehman et al. (1991). The average follow-up period was 1-3 years. The results were satisfactory in 38.8% of cases and unsatisfactory in 61.2%. The possible causes of unsatisfactory results in our series involved the following factors : 1- Age at Operation: The best results were obtained in children who were operated on when they were 6 months old or less. At the time of operation, 49% of our patients were older than 6 months. 2- Postoperative Immobilization Period: The best results were obtained when postoperative immobilization period was> 2 to 4 months (43% of feet). Less satisfactory results were found in the remaining 57% of cases when the postoperative immobilization period was 2 months or> 4 months. 3- Previous Treatment: The presence or absence of history of surgery prior to Turco’s operation was not an important factor in predicting the final results as we had anticipated. This is due to the fact that 62% of the feet with previous surgery had had a prior posterior release only with a lesser amount of scar tissue than in the remaining feet in whom a prior posteromedial release was attempted.
