Abstract

Myocardial ischemia and lor infarction in children may be under ­estimated and is likely to be much more common than is generally believed. This may be related to unawareness of the different clin.ical presentations of myocardial ischemia in children which are quite different from those in adults being non specific symptoms or signs. Also the lack of performance of specific investigations for diagnosis may be another factor. Unfortunately myocardial infarction in children may be only diagnosed after death at autospy. Therefore, early diagnosis necessitates a high index of suspicion considering that myocardial ischemia in children is not always a remote possibility. However, there are certain high risk groups of pediatric patients associated with increased incidence of myocardial ischemia or infarction as patients with critical aortic stenosis, congenital anomalies of the coronary arteries, MVP, HeM, kawasaki syndrome, systemic diseases as diabetes mellitus; SLE, PAN, sickle cell ane1llla and familial hypercholesterolemia. There are certain dangerous warnmg symptoms which may be considered predictors of early myocardial ischemia such as typical anginal pain, syncope, palpitations, dyspnea, and easy fatigability.
