Abstract Pancreatic carcinoma is the fifth leading cause of cancer death in the United States (Parker et al, 1997). In this study 120 patients underwent pancreatico duodenectomy for periampullary carcinoma, 36 (52.5%) pancreatic carcinoma, 41 (37%) ampullary carcinoma, 7 (5.8%) carcinoma of the lower third C.B.D., 5 (4%) duodenal carcinoma and 4 (3.7%) chronic pancreatitis, 81 (68%) were subjected to pancreatico gastrostomy and 37 (31 %) were subjected to pancreatico jejunostomy. Hospital mortality was occurred in 5 (4%) patients and early post operative complications in the form of delayed gastric emptying 18 (18%), wound infection (12 (10%), pancreatic fistula, 8 (6.6%), bile leak , 7 (5.80/0), bleeding 8 (6.6%), peptic ulcer 10 (8.5%) and intra-abdominal abscesses (7 (5.8%). Late mortality was occurred in 54 (45%) patients with recurrance related mortality was in 45 (37%), cardiac problems in 5 (4%) and 5 (4%) were of unknown cause. Recurrance was detected in 58 (48%) of total population with 19 (32%) were in form of liver metastases, 22 (38%) local recurrence and 17 (29%) recurrence in lymph nodes. |
