Abstract

Congestive heart failure (CHF) is a major cause of cardiovascular mortality and morbidity all over the world. A wide variety of mediators are involved in the pathophysiology of heart failure. One of these mediators is adrenomedullin (AM), which is a vasodilator and hypotensive peptide recently discovered from the tissue extract of pheochromocytoma. The present study has been carried out to: 1. Determine the changes of plasma adrenomedullin in CHF patients with different aetiologies. 2. Determine the changes of plasma adrenomedullin in response to treatment of CHF patients for 2 months. 3. Explore the relations between plasma adrenomedullin and some neuroendocrine hormones involved in the pathophysiology of heart failure involving plasma renin activity, atrial natriuretic peptide and aldosterone as well as some echocardiographic parameters in CHF. 64 patients with symptomatic congestive heart failure NYHA class (I-IV) have been enrolled in the present study, of them 440/0 females and 56% males, their mean age was 41.37± 9.38 years. Fifty percent of the patients were in NYHA class I-II and fifty percent were in NYHA class III-IV.
