Abstract

The present study was conducted to asses the value of CT Urography in detection of non calcular causes of upper urinary tract hematuria which varies from tumors, trauma to vascular causes. The material consisted of 24 cases presented with hematuria, 18 were males, 6 were females, their age ranged from 8 to 76 years with mean age of 48 years cases with stones or bladder causes were excluded. Clinical and radiological examination were done for all patients. The following radiologic investigations were alone for all patients. - UT Plain - Ultrasonography - Excretory urography (EU). - CT urography. - CT urography technique was followed by (EU) to take advantage of injected IV contrast medium and no oral contrast was used spiral CT was done from the upper pole of kidneys to the symphysis pubis 15 cases in this study found to have renal masses; 13 cases of parenchymal tumors, 2 cases of pelvicalyceal tumors 2 cases were found to be benign masses (one case of angiomyolipoma and the other xanthogranulomatous pyelonephritis. All masses were about 3 cm or more in diameter, so were detected by US as well as CT but CT urography added more characterization, helped in staging of malignant masses and detected vascular invasion in 3 cases. CT urography was helpful in diagnosis of 2 cases of ureteric tumors in which IVP detected back pressure changes and filling defect in one unit but did not detect the tumor in the other unit because the kidney was not functioning. US was inconclusive in detection of ureteric tumors. - 2 cases of renal trauma were diagnosed by CT Urography and detected perinephric collection in one of them vascular phase of CT urography was able to diagnose vascular anomalies as retro aortic left renal vein (Nutcracker syndrome). From this study we conclude that in cases of hematuria the following radiologic investigations should be followed - UTP for detection of urinary stones. - US for detection of stones or renal masses. - For non calcular cases, early and delayed post contrast spiral CT for better detection of renal masses, vascular causes, renal trauma or inflammatory causes followed by EU using the same injected contrast media for better depiction of renal function, ureters and urinary bladder.
