Abstract

The augmented valved rectum and double folded rectosigmoid bladder represent two modifications to circumvent the inherent problems of ureterosigmoidostomy. The augmented valved rectum has the advantage of functional isolation of the reservoir through construction of a colorectal valve. In this study, we present the long-term outcome analysis of these procedures in a pedlatric population. The long,.term effect on upper urinary tract, continence, metabolic profile, linear growth and bone density was studied. Between March 1987 and May 1997, 28 boys and 13 girls underwent urinary diversion to a low-pressure rectal reservoir at Ma,nsoura Urology & Nephrology Center. Age at surgery ranged from 2 to 14 years (mean 6.3). The augmented valved rectum technique was utilized in 26 cases and the double folded rectosigmoid bladder method was used in 15. Follow-up included supine height measurements, ultrasounds and/or intravenous urography, voiding proctography, bone mineral density measurement, serum electrolytes, and arterial blood gases. Statistical analysis was conducted utilizing binary .logistic regression, student’s t-test, and Fischer’s exact tests. Two patients died secondary to metastatic rhabdomyosarcoma. The remaining 39 patients were evaluable with median follow-up of 60 months (range 24-148). All
