Abstract

The Ilizarov method is a revolutionary option in treatment of tibial nonunion. It provides a useful and almost irreplaceable tool for the treatment of the most difficult cases, particularly those complicated with infection, deformities and loss of bone substance. In this study, 21 cases of tibial nonunion have been reviewed. 4 females and 17 males, the median age was 35 years, the median duration of nonunion was 13 months, and the median preoperative leg length difference was 3 em. According to the radiological appearance and the clinical examination, 8 cases were classified as atrophic (lax), 13 as hypertrophic (stiff). There were 7 cases with pre operative deformities. There were 12 (57%) infected and 9 (43%) non-infected cases The treatment methods used in this study were either monofocal treatment, which was mainly used for treatment of stiff nonunion, and multifocal method which was mainly used for the treatment of lax nonunion, for the infected cases, or when LLD was greater than 3 em. The monofocal treatment was used, however, for the treatment of some of the less severely infected cases. It was successful in achieving union and eradication of infection but with a longer duration of fixation than the non-infected cases treated by the same modality.
