Abstract

Holmium: Y AG laser has been introduced as a new technology for stone disintegration. It acts mainly via thermal effect resulted in stone melt down into fine dust. Swiss lithoclast is another established modality for intracorporeal . stone fragmentation. It acts via mechanical effect on the stone ”Jack hammer action” resulted in fragmentation of the stone into sizable pieces. 80 patients with ureteral stones at different level were included in our study. They were prospective.]y randomized between Holmium: Y AG laser and Swiss lithoclast as two recent modalities for intracorporeal lithotripsy/each group contained 40 patients. Overall success rate in laser group was 90% and in lithoclast group was 82.5%. Incomplete disintegration was reported in 7.5% of laser group and in 17.5% of lithoclast group. While failure was reported only in laser group (2.5%). The comparison between laser and lithoclast was found statistically insignificant. . Complication rate of laser group was 10% and in lithoclast group was 12.5%. . .After review of literature and based on our results, we concluded that both Holmium: Y AG laser and Swiss lithoclast are safe, effective and reliable modalities for intracorporeal stone disintegration. However, upward stone migration is reported more with lithoclast application. Lithoclast is associated with higher rate of auxilIary procedures application in comparison to laser. Holmium: Y AG laser is costy, however, it is a multipurpose device. i.e. it can be used for management of many urological diseases rather than urinary stones as ureteral strictures, urethral strictures, ablation of urothelial tumours and enucleation of the prostatic adenoma. In addition, Holmium: Y AG laser is a multispeciality device i.e. it has many applications in medicine like orthopedics.
