Abstract

Introduction and aim of work: Cystectomy with urinary diversion with continent urinary diversion. These types of diversion were used in our study with comparison between them. Patients and Methods: Between 1996 - 2001 a total of 30 patients underwent radical cystectomy for cancer bladder with three different modalities of urinary diversion. - Mean age of patients was 52 years. - Male to female ratio 22 : 8. - Preoperative assessment of patients were done. - Postoperative assessment of patients were done. Then evaluates of patients regarding to: - Continence. - Number of urination at day and night. - Stress incontinence. - Soiling and its degree. Patients were classified into three groups: - Group I modified Camey. - Group II Kock urinary pouch. - Group III Sigma rectum pouch. Results: The results of study shows that the different types are closed to each other in their advantage and complications but sigma begin to be superior in relations to the other types. Conclusion: Modified Camy is simpler than Kock’s pouch but Kock seems to be slightly better as regard reflux and continence. Modified sigma pouch has the advantage of less operative time effective continence (controlled by strong anal sphincter), less metabolic complication. In male patients the site of the tumour might be taken in consideration if within 3cm from ureteric orifices. So the radical cystoprostatecotmy with sigma pouch is safe to avoid local recurrence. In female patients the modified sigma pouch is an ideal method of diversion for better continence and radicality.
