Abstract

Carcinoma of the stomach continues to be a significant health problem around the world, despite the fact that it has continued to decline in incidence. Many factors were associated with a higher incidence of gastric cancer such as: smoked or salted foods, foods contaminated with aflatoxin, low intake of fruits and vegetables, low socio economic status, and possibly a decreased use of refrigeration, possible occupational relationships include cool mining and rubber or asbestos workers, precursor pathologic conditions include pernicious anemia, achlorhydria, atrophic gastritis, gastric ulcer and adenomatous polyps. Also several studies have shown a three to six times increased risk of gastric cancer in patients with Helicobacter prylori infection versus those with no infection. The term gastric cancer usually refer to adenocarcinoma, since this histology accounts for 90 to 95% of all gastric malignancies. This study was performed on 197 gastric cancer patients who were referred to Clinical Oncology and Nuclear Medicine Department outpatient clinic, from Surgical Gastroenterology Center and General Surgery Department, Mansoura University Hospital, during the period from 1991 - 2001 inclusive. Conclusions: ” Early detection help to predict favourable outcome and progression. ” Management of gastric cancer relies primarily on surgical resection of the involved stomach. However, trials still the only way of developing future standardized treatments without exposing our patients to unproven and thus potentially dangerous treatment fads. ” Combined modality treatment, surgery followed by chemoradiotherapy has the best results in treatment of gastric cancer and results in longer survival duration. ” Regular follow up and endoscopic examination are mandatory for early detection of recurrences and for proper management of disease failure. ” Further studies with higher numbers of patients are needed to offer the best treatment modalities. ” An attempt to use newer drugs or modalities such as gene therapy, vaccines and antiangiogenesis in treatment of gastric cancer patients.
