Abstract

The present study was conducted on 30 patients suffering from CRF. On clinical background and ECG, none of these uremic patients was in acute ischemic attack. Each patient and control subject was suspected to full history taking, clinical examination, ECG, routine laboratory investigations and measurement of plasma levels of troponin, homocysteine, NO metabolites and endothelin. The present study reports the following results: Lipogram parameters: ” There is significant increase in cholesterol level in HD group in comparison to control group. There is significant increase in triglycrides and LDL-C levels in both conservative and HD groups in comparison to control group. On the other hand, HDL-C level and HDL/LDL ratio show significant decrease in both conservative and HD groups in comparison to control group. However, all lipogram parameters show insignificant difference between HD and conservatives. ” The comparison between conservative group with risk and control group shows significant increase in triglycrides and LDL-C levels and significant decrease in HDL-C and HDL/LDL ratio but there is insignificant changes in cholesterol level. However the comparison between conservative group without risk and control group reveals insignificant change in all parameters. The comparison between the two patients groups reveals significant increase in LDL-C and significant decrease of HDL/LDL ratio in conservative group with risk. Cardiac markers: ” There is significant increase in CRF patients either with risk factors or without when compared to healthy control group. There is insignificant difference either between the conservative and HD groups or between CRF patients with risk and those without. On the other hand, serum CK-MB levels show insignificant difference between CRF(either conservative or HD) with or without risk factors and healthy control group or in between the patients groups ” There is significant increase in the CnI level in HD group either with risk or without when compared to control or conservative group. There is insignificant difference between conservative and control groups. Endothelial markers: ” There is significant increase in the measured endothelial markers (homocysteine, NO and endothelin) in both conservative and HD groups in comparison to the control group. But the mean values of these parameters increase insignificantly in HD group if compared to the conservative group ” There is also significant increase in all measured endothelial markers in conservative or HD groups either with or without risk factors in comparison to the control group. The difference between the two patient groups is non significant. ” Comparison of CRF group with HTN and CRF group without risk shows significant increase in endothelin level. ROC Curve: ” Comparison of the sensitivity and specificity of the studied cardiac and endothelial markers in CRF patients using the ROC curve(Figure 14) demonstrates that CnI is the most sensitive and specific marker for detection of cardiac risk in those patients(AUC= 0.920), followed by endothelin , CK , CK-MB, homocysteine and NO. At a cut off value of 0.33 ng/ml ,CnI has sensitivity of 83.3% and specificity of 87.5% with positive predictive value(PPV) 62.5% for prediction of cardiac risk.
