Abstract

This study was carried out on 60 male patients ASA I, II aged more than 60 years undergoing a unilateral orthopedic surgery involving the lower limbs in Emergency Hospital ,Mansoura University .. The patients were randomly classified into 2 groups: Group I (n=30) : 2.5 ml of hyberbaric bupivacine 0.5% (12.5 mg) was injected in 2 - 3 seconds (fast group) and Group II(n=30) : 2.5 ml of hyberbaric bupivacine 0.5% (12.5 mg) was injected in 2 min. (slow group) . The range of maximal sensory level obtained 15 min. after intrathecal bupivacaine injection in ipsilateral sides of fast and slow groups was T4-T10 and T4-L1 respectively ,the corresponding readings in contralateral side were T6-L3 and T4-L3 respectively . When compared the sensory level in ipsilateral and contralateral sides in the same group we observed significant changes (p=0.005 and 0.004 in fast and slow groups respectively ) . After 15 min . from spinal anaesthesia ,24 (80%) and 23 (76.6%) patients of fast and slow groups respectively showed complete motor block in contralateral limb. Heart rate readings remained on the high normal values in both studied groups throughout the study period . Mean blood pressure was kept at an accepted range for the geriatric patients (74-90 mmHg) in both groups with no significant difference between them . from this study we can conclude that regardless the speed of injection , there is no differences in the anaesthetic characteristics of spinal anaesthesia both of them are reliable for clinical practice in elderly patients.
