Abstract

Ucerative colitis is wide spread in distribution and varies in its incidence. It is a relapsing and remitting disease characterized by acute non infectious inflammation of the colorectal mucosa which is variably affected by a confluent inflammation and shallow ulceration extending proximally from the anal margin. The most feared long term complication of ulcerative colitis is cancer which is closely associated with dysplasia Purpose: Histopathological study and evaluation of ulcerative colitis cases including the degree of activity, chronicity, the presence or absence of dysplasia and its degree ­ A trial to apply a new recent grading system of activity in ulcerative colitis cases <U+2013> Assessment of the proliferation potential and gene alterations in cases with dysplastic lesions. Materials and method: The present study was performed on 99 colorectal biopsy specimens. They were collected from Gastroentrology center, Mansoura university in a retrograde manner. The slides are classified into two groups; the first was ulcerative colitis cases (74 cases) and the second was dysplasia with ulcerative colitis (25 cases). The cut slides were used for staining for routine H&E stain as well as immunohistochemistry for ki­67 and p53 in the dysplastic cases. Results: 11 cases are excluded from the 74 ulcerative colitis cases and they can be considered as chronic non specific colitis. They are classified according the grading system of activity, which includes six grades. The dysplasia with ulcerative colitis cases is classified into 12 cases indefinite for dysplasia. 6&3 low and high grade dysplasia while 4 cases are lasted in the preparations <U+2013> Males are more common than the females <U+2013> The incidence is found in the 3rd decade­ The most frequent clinical symptom is bleeding/ rectum while the most frequent endoscopic findings is severely inflamed mucosa <U+2013> The pattern of staining of both ki67 and p53 are differ according to the type of dysplasia; in low grade dysplasia it is localized in the lower two third of the crypts while it extends to top and surface area in high grade dysplasia. Conclusion: Ulcerative colitis grading system of activity must be applied in every case <U+2013> The cases of ulcerative colitis with dysplasia must have routine ki­67and p53.
