Abstract Liver resection remains the only potentially curative treatment for liver tumours. Major hepatic resection have been associated with in creased introoperative blood loss and this leuds to operative morbidity and postoperative complications 4 patients with conditions awenable for hepatic resection were subdivided into 2 groups cach of 20 patients. group I have done conuentional hepatic resection & group II done hepatic resection with hypotensive anesthesia. Aim Of The Work : The aim of this study was to evaluate the effect of using hypotensive anesthesia on liver function in patients undergoing hepatic resection and the benefits from this new technique as regard operative time, operative morbidity, decreased risk of blood loss, coagulopathy, transmission of diseases , unwanted reactions. RESULTS: Our results shows that there is significant decrease in blood loss, transfusion requirements and mean operative time in the hypotensive group. Biochemical parameters recorded at the preoperative period (basal), first third seventh and 14th days postoperatively shows that there is an alteration in liver function rests in the two groups and this alteration improved gradually postoperatively at the 7th day . SUMMARY AND CONCLUSION :from this study we can conclude that hepatic resection with the use of hypotensive anaesthesia together with the use of ligasure and tissue link can achive the dream of blood less field and it resulted in reduced operative time, amount of blood loss and decreased the need for blood transfusion with its complications. |
