Abstract

Patients & Methods : The study included 118 cases of nasolacrimal duct obstruction in 118 patients that divided into two groups (prospective & retrospective) who underwent El­Mansoura DCR operation between October 2003 and August 2004. Full history taking and full ophthalmologic examination with special attention to the lacrimal system were done to all cases. Dacryocystogram was also done to all cases. All cases were undergone physiological, anatomical and clinical assessment to evaluate success of El­Mansoura DCR operation. All cases were undergone postoperative endoscopic examination at 3 months and 6 months for prospective group and once for retrospective group. C.T. helped us to measure diameters of the bony opening of 16 selected cases of prospective group Results : The different methods used for the evaluation of El­Mansoura DCR revealed that the physiological success rate among the studied cases was 94.4 % as indicated by the FDD test. Anatomical success was 96.6 % as tested by the irrigation test. Clinical success was 94.4 % as indicated by resolved epiphora and negative regurge test. Our results failed to support the theory that a larger surgical resection results in a significantly larger permanent intranasal ostium. Endoscopic examination revealed that the most common causes of failure are an in­ appropriate size and site of the osteotomy by granulation tissue. Conclusions : El­Mansoura DCR is a simple technique with a higher success rate. So, It is considered as a good alternative procedure to the traditional external DCR that overcame most of its difficulties. Nasal endoscope has a great promise as a diagnostic and therapeutic instrument. Computed tomographic examination gives a good idea about the site and size of postoperative bony opening.
