Abstract

This study was conducted on a total of 92 cases of ED. All cases were subjected to : thorough history taking and clinical examination , measurement of penile aspect ratio, percent stretch and percent recoil of penile tissues , measurement of blood glucose, serum total testosterone and prolactin levels . The IIEF score for every patient was recorded. Pharmacologic-induced erection were obtained for all cases by ICI of 60 mg papaverine , ICI of 20 g PGE1 and 50 mg oral Sildenafil with MSS . Quantification of erectile response were done by clinician judgement ( E0 - E5 ) , measurement of penile axial rigidity , ICP and glans-penis temperature by DIR , measurement of cavernosal arterial dilatation , PSV, EDV and RI by colour duplex and measurement of penile tumescence and TAU, radial rigidity and RAU values by Rigiscan- Plus. The severity of ED were classified according to the IIEF scores into: mild - mild to moderate - moderate and severe ED . Reproducibility of the response to papaverine , PGE1 and oral sildenafil were tested for 7, 7 and 9 cases respectively that were randomely selected among the whole 92 cases. It was found that ICI of 20 g PGE1 was the most effective drug for stimulation of penile erection compared to ICI of 60 mg papaverine and oral intake of 50 mg sildenafil with MSS in terms of examiner’s assessment, DIR, Rigiscan -Plus and PPDU . Significant correlations were found between the penile axial rigidity, radial rigidity and penile hemodynamics specially the RI . No significant correlation was found between the IIEF score and the DIR , colour duplex and Rigiscan-plus results. DIR parameters , colour duplex parameters ( except cavernosal arterial dilatataion ) and Rigiscan -plus parameters were found to be reproducible after different pharmacologic interventions . Reasonable sensitivity , specificty and diagnostic accuracy were found for RTM Rigiscan-plus) , DIR and EDV and RI but not for PSV and cavernosal arterial dilatation (colour duplex).
