Abstract

Myocutaneous flaps are based upon the principle that in most regions of the body the skin derives its blood supply not from specific cutaneous vessels, but rather from multiple small vesselS passing to the skin from the underlyi~g muscle. Since most of the muscles in the body are nourished by a discrete number of easily identified arteries, it is possible .,; to elevate a given muscle in conjunction with the overlying skin as a unit, with both skin and muscle receiving their blood supply from the primary muscular vessels. Myocutaneous flaps have several advantages over random skin flaps, tubed pedicle flaps, free flaps and other techniques. They can be transferred directly to the recipient site in a single operation, avoiding the long ~ repetitive hospitalisations characte~istic of tube pedicles, and circumventing the known uncertainties associated with random skin flaps. They can provide weI-vascularized tissue, which can be expected to be beneficial for healing of i~,;;cl’ic:E~mic, inf0?cted or il”Tadiated ti!:>suE’~’ A significant consideration is that the elevation and transfer of a myocutaneous flap is not technically difficult, is easily learned and does not depend upon the maintenance of the high level of laboratory proficiency required for free flap tr”;;:n~;f 0?r~:,. Under certain circumstances, a myocutaneous flap can incorporate ~ segment ,of bone for osseouS reconstruction. Clearly, their
