Abstract

This study was done to evaluate the effect of topical application of the fibrin glue on the healing of dental socket in guinea pigs histologically and histochemically.48 adult guinea pigs of average weight 300- 400gm were divided into control group and experimental group as followed: Control group (group I): 18 animals were subjected to general anesthesia using Thiopental sodium (Intraval) via intraperitoneal injection at 50 mg/kg body weight. An intramuscular injection of 25-50 mg/kg Cefoberazone sodium (Cefobid) was administered for perioperative antibiotic prophylaxis. The gingiva surrounding the mandibular first molar tooth was dissected and luxation of the tooth by enamel hatchet was made. After that, the tooth was extracted with some cortical bone using a curved artery clamp and bone margins were curetted with a small bone curette to remove any reminant of fractured root. The dental socket was handled with pressure to stop bleeding with a gauze sponge. After achieving hemostasis, approximation of the wound surfaces were accomplished using 4/0 silk suture. The sutures were removed at seventh postoperative day. The control group was divided into three subgroups each containing 6 animals, as following:- Subgroup A: 6 guinea pigs were sacrificed at 2nd week. Subgroup B: 6 guinea pigs were sacrificed at 4th week. Subgroup C: 6 guinea pigs were sacrificed at 6th week. Experimental group (group II): 30 animals were treated in the same way as in the control group but fibrin glue was used instead of 4/0 silk suture for wound closure. The experimental group was divided into three subgroups each containing 10 animals, as following: Subgroup A: 10 guinea pigs were sacrificed at 2nd week. Subgroup B: 10 guinea pigs were sacrificed at 4th week. Subgroup C: 10 guinea pigs were sacrificed at 6th week. The histochemical examination revealed the following: • Deposition of PAS positive materials were observed to be increased in the groups that had topical application with fibrin glue in a short time compared to the sutured groups. • Alkaline phosphatase activity was stronger in the groups that had topical application with fibrin glue in a short time compared to the sutured group. • Deposition of Alcian blue positive materials were observed to be increased in the groups that had topical application with fibrin glue in a short time compared to the sutured groups. The statistical results revealed the following: There is no significant difference in the average bone formation between the experimental and control sub group in the 2 weeks period. There is significant difference in the average bone formation between the experimental and control subgroup in the 4 and 6 weeks period meaning that the amount of bone was increased significantly when using fibrin glue. Conclusion: Based on the previous results, it can be concluded that : • The activity of osteoblasts increases by using fibrin glue. • The effect of fibrin glue was not significant after 2 weeks in healing of the dental socket. • Complete closure of the dental socket occurred after 6 weeks. Fibrin glue repair is the experimental method that has more advantages in healing of the dental socket after tooth extraction as compared with conventional suture repair.
