Abstract

Introduction: Colorectal cancer is a major cause of mortality worldwide. In the United States, it is the third most common type of cancer and is the second leading cause of cancer death, with approximately 145,000 new cases and 56,300 deaths projected for 2005. Progress in identifying and understanding the molecular and cellular causes of cancer had led to the discovery of anomalies that characterize cancer cells and that represent targets for the development of cancer therapeutics. One such target is the epidermal growth factor receptor (EGFR). Aim of work: To explore the incidence of EGFR expressed in patients with colorectal cancer. Secondly correlate cases with expressed EGFR with other known prognostic factors. Lastly to identify those subsets of patients over-expressed who may got benefit from giving them unconventional treatment protocol. Patients and methods: Prospective study included fifty patients histologically-confirmed colorectal adenocarcinoma, operable, stage T2-4, N1-3, as determined by post-operative pathologic examination, no evidence of distant metastasis, and no evidence of previous chemotherapy. For whom Assesment of EGFR on the paraffin block. Patients who expressed EGFR divided into two arms, one received conventional treatment and other received intensive treatment (FOLFOX or FOLFIRI). Results: EGFR were positive in 60% of patients and –ve in 40%. There was significant relation between EGFR expression and site of disease and incidence of recurrence. Patients received intensive treatment achived better DFS and OAS. Conclusions: EGFR expression affect on incidence of recurrence. Patients with expressed EGFR should receive intensive treatment.
