Abstract

Introduction: Although several studies used soft liner as a female housing over the bar or over the ball attachments to retain overdentures on the implants, however neither of these studies were concerned with evaluation of the effect of this type of attachment on peri-implant tissues. Aim of the study: This study was aimed to evaluate the effect of different manners of denture base-tissue contact on peri-implant tissue changes for implant retained mandibular over denture. Materials and methods Twelve completely edentulous male patients complaining from reduced stability of their lower denture were randomly grouped as follows: Group I: have full denture acrylic contact anteriorly and posteriorly, Group II: have full denture soft liner contact anteriorly and posteriorly., Group III: have the distal extension base left as an acrylic mucosal contact and the denture retained anteriorly to the bar by soft liner , Group IV: have the distal extension base of the denture lined with soft liner and the denture was retained with clip to the bar anteriorly. Peri-implant tissue changes was evaluated; clinically in terms of peri-implant modified plaque and bleeding indices, peri-implant gingival index and peri-implant probing depth, radiographically in terms of peri-implant alveolar bone changes and immunologicaly in terms of interlekin 1-β. Results: The results of this study demonstrated that; wearing of mandibular overdenture with soft liner tissue contact either anteriorly over the implants and posteriorly over the distal extension base or anteriorly over the implants only significantly decreased peri-implant plaque scores, bleeding scores, gingival scores, probing depths, vertical and horizontal alveolar bone loss and interleukin 1-β in peri-implant crevicular fluid with advance of time than that reported with patient wearing mandibular overdenture with hard acrylic denture base tissue contact either anteriorly over the implants and posteriorly over the distal extension base or anteriorly over the implants only. Conclusion: Lining of the overdenture with soft liner either aneriorly as female housing for bar retention only or anteriorly and posteriorly to obtain intimate contact of the distal extension to the underlying posterior edentulous area objectively improves the health of peri-implant tissues.
