Abstract

Recently, it has been recognized that children and young adults with CKD are at significantly increased risk of cardiovascular morbidity and even mortality compared to the general pediatric population. Earlier detection of cardiovascular abnormalities in these patients might allow earlier interventions to reduce morbidity and mortality. This study was conducted to assess cardiac functions in children with chronic kidney disease attending Nephrology Unit, Mansoura University Children’s Hospital in order to: Determine the magnitude of cardiovascular disease among these children with CKD and identifying the cardiovascular risk factors in these patients. Forty children suffering from chronic renal failure participated in this study with their parents’ oral consent. They were divided into 2 groups; Group I (Dialysis group) included 20 children with ESRD on regular hemodialysis and Group II (Pre-dialysis) included 20 children. A group of 20 apparently healthy, age and sex matched children was served as a control group. Results revealed: Not only children receiving chronic dialysis, but also children with mild or moderate CKD had abnormal cardiac functions. Left ventricular global performance is impaired in pediatric patient with chronic kidney disease. LV systolic function is normal, while diastolic function is impaired in these children. Global right ventricular performance as well as diastolic function is impaired in pediatric ESRD. CK-MB is a sensitive marker for cardiac injury in children with chronic kidney disease. Dialysis, hypertension, anemia, renal oesteodystrophy, acidosis, and hyperlipidemia are risk factors for development of cardiac abnormalities.
